The Opportunity: As a Support Coordinator at Sakura Connect, your role is instrumental in assisting individuals with disabilities to navigate the National Disability Insurance Scheme (NDIS) and access the supports they need. This role involves facilitating the coordination of services, empowering participants to achieve their goals, and ensuring they receive appropriate funding and community supports.
Responsibilities:
- Participant Support: Provide one-on-one support to NDIS participants to help them understand and implement their NDIS plans.
- Goal Achievement: Collaborate with participants and their families to identify and work towards individual goals and aspirations.
- Service Coordination: Assist in connecting participants with service providers and community resources that align with their goals.
- Funding Management: Monitor and track NDIS funding to ensure it is used effectively in line with participants' preferences.
- Advocacy: Empower participants to advocate for their rights, choices, and needs within the NDIS system.
- Report Preparation: Maintain comprehensive records and prepare reports to document participants' progress and funding allocation.
- Crisis Response: Provide support during crisis situations, including coordinating emergency services if necessary.
- Collaboration: Liaise with service providers, allied health professionals, and other stakeholders to ensure a coordinated approach to support.
- NDIS Compliance: Stay informed about NDIS policies, regulations, and relevant legislation to ensure compliance.
- Cultural Sensitivity: Work with participants from diverse backgrounds in a culturally sensitive and respectful manner.
Qualifications:
- Tertiary qualification in a relevant field (e.g., Psychology, Social Work, Occupational Therapy) or equivalent experience in the disability or community services sector.
- Knowledge of the National Disability Insurance Scheme (NDIS) and relevant legislation, including the NDIS Act 2013.
- Strong communication skills, both written and verbal, for effective interaction with participants and stakeholders.
- Demonstrated ability to build positive relationships with participants, families, and service providers.
- Problem-solving skills and adaptability to handle various situations and crisis responses.
- Empathy and an understanding of psycho-social disabilities and the mental health system within the NDIS framework.
- A commitment to a person-centered approach, respecting participants choice and control.
- Flexibility to work within a changing environment.
Screening Criteria:
- National Police check.
- Working with Children check and NDIS Worker Screening check, or the ability to obtain them.
- Valid driver's license if required for the role.
- Right to work in Australia.
- Up-to-date with COVID-19 vaccinations or valid medical exemption if applicable.
